Submit all of the following for Admission Consideration:

1. Complete and sign the St. John Paul II Catholic High School Online Application

2. Submit the following required documents:

– Your child’s most recent report card or transcript including attendance records. Please indicate if your child will complete End of Course exams for any subjects prior to the start of the next school year if not listed on his/her transcript.

– Standardized Testing Results from Middle School years (6th-8th) or most recent High School Standardized testing result. Include any End of Course exam results (Algebra 1, Biology, Geometry).

– If eligible for special services, submit copies of 504 Plan and/or IEP and Matrix (required to be submitted if applicable).

– A copy of your child’s Birth Certificate.

– A DH680 (Immunization Form) and DH3040 (Florida School Entry Health Exam).

– Two references from a teacher or administrator. At least one of the references must come from a core subject teacher (English, Math, Science, Social Studies, World Language). References must be completed by the teacher or administrator and submitted directly through a link provided in your application.

3. Schedule a time to take a placement test.  We use this standardized test to supplement the transcript and for course level placement should the student be admitted.

After Acceptance:

1. Pay the $250 Enrollment Fee via check, cash, or online payment. Click here to make payment online.

2. Register online for FACTS tuition payment plan

3. Submit a Baptismal Certificate for your child (if Catholic) for the Catholic Grant.

4. Submit any Athletic forms (EL2/EL3 & GA4) to our Athletics department.

5. Submit signed Conditional Acceptance Letter (if applicable).

Waiting List Information:

We are unable to accept every applicant to our school and, in some cases, may place students not admitted on a waiting list. A student’s likelihood of being admitted from the Waiting List will be dependent on vacancies and course placement. The Waiting List is not ranked and is kept active all year. We accept applications all year, even if the school does not have vacancies.

Athletes may not practice with a sports team unless acceptance has been issued.
